At first appointments and when you need to request release of confidential information, you may be asked to provide detailed information and consent.  The documents below can be printed, filled in and brought to your appointment.  This will save you time and effort in completing these forms at the time of your appointment at any of our HeartCare Midwest offices or regional clinics.
